This isn’t “move fast and break things.” This is “move carefully and document everything.”
Dreamforce Healthcare Use Cases
1. Patient Intake and Scheduling Agent (Kaiser Permanente)
What it does:
- Conversational agent for appointment scheduling
- Symptom assessment and triage (non-diagnostic)
- Insurance verification
- Provider matching based on patient needs
HIPAA considerations:

What it does:

- Escalates to pharmacist when needed

Clinical Decision Support: The FDA Problem
This is where most healthcare organizations get stuck.
FDA’s position (updated 2024):
- AI that informs clinical decisions = lower risk (guidance, not approval)
- AI that makes clinical decisions = Software as Medical Device (requires pre-market approval)

Human approves, but was decision truly independent?
Kaiser Permanente’s approach: Agents always recommend, never prescribe. This keeps them FDA-exempt while still valuable.

The Bias and Equity Problem
Healthcare AI faces serious algorithmic bias concerns.
Pfizer’s learnings:
- Early clinical trial matching agent over-recommended trials to white patients (training data bias)
- Agent learned from historical trial enrollment data (which was non-representative)
- Had to retrain with demographic fairness constraints
Mitigation strategies:
- Demographic monitoring: Track agent recommendations by race, gender, age, zip code
- Fairness metrics: Ensure equal recommendation rates across demographics
- Human review: Clinical teams review agent outputs for bias monthly
- Diverse training data: Intentionally oversample underrepresented populations
